Chart Review Database Attributes

The Chart Review Database is a Microsoft Access-based improvement tool that screens for severe sepsis, collects and organizes data for chart abstraction, and calculates the monthly results of each of the Severe Sepsis Quality Indicators.  Enter the results of your chart abstraction and the database automatically generates graphs of your improvement and produces flow charts documenting the results of your resuscitation efforts.

**Note: the glucose control indicators have changed, however the database has not been upgraded to re-calculate for glucose less than 180 as the new recommended range by the Surviving Sepsis Campaign.  Presently, there is no plan to upgrade the database.  Please refer to the appropriate measures pages regarding glucose to configure a manual measure for glucose control.

 
Chart Review Database Attributes

Microsoft Access 2003 database with Microsoft Acess runtime multiuser - no connection to SQL server. 

REQUIRES:

Windows 2000 or XP

  • Microsoft Office 2000, 2002, 2003 installed with Microsoft Word and Microsoft Excel Arial Fonts
  • Microsoft Access Run Time (provided with download be sure it isn't blocked by firewall)
  • Internet access for data reporting functions

The database is free and such very limited support is available. Please read all documentation before installing. More extensive data support is available however charges may apply.

To download the Surviving Sepsis Campaign Database directly from this site first read the chart review database FAQ sheet and also note you must register (step 2) to receive the installation password:

STEP 1:    Install Database Application

  • Click the link above to download the Surviving Sepsis application. Use WinZip to unzip the file, then run the installation by double clicking the SSCDB icon from within the unzipped folder.

STEP 2:  Register

You will receive a numeric passcode. This will come to you via email. Store this numeric code in a safe location as it is not retrievable from SCCM. The code identifies your hospital and you as a contact.


STEP 3:  Upload 4.0 Upgrade (see upgrade description below)

  • Click the link above to download the latest version of the database.
  • Use Winzip to unzip the file, then run the installation by double clicking the SurvivingSepsis.MDB icon.
  • Use the Search feature in Windows (Start --> Search) to locate the folder named SepsisDB. The most likely path for this folder is "C:\SepsisDB" but if you have installed on a network drive may be another drive such as "S:\SepsisDB."
  • Drag the unzipped file SurvivingSepsis into the folder SepsisDB wherever it is located.
  • The computer will ask you if you want to overwrite the existing file. Agree to overwrite the file.
  • Next, go to your desktop icon and open the sepsis database. The database will notify you, "The links are broken, do you want to re-establish the links?" Agree to re-establish the links.
  • The database should open normally and all of your data will be preserved.
IMPORTANT Note Regarding V4.0

It has been recently found that there is an error in the database that applies to sites that are collecting data using the units mg/dl. Most sites collect lactate using mmol/L as the unit of measure. This is true for instance in nearly all sites in the United States. The error results in your data showing a poorer performance than you should expect with respect to the CVP and ScVO2 measures in particular. Since 4 mmol/L is equivalent to 36 mg/dl, the error results in sites using mg/dl as the unit of measures in particular. Since 4 mmol/L is equivalent to 36 mg/dl, the error results in sites using mg/dl as the unit of measure more frequently being scored as failing to check CVP and ScVO2 when indicated. In truth, the impact is likely to be minimal on your performance however; it is important for your own results at home to complete the upgrade. Once you have upgraded your database to version 4, re-transmit your data to the SSC. We need you to do this to ensure that we have accurate data. We are able to manipulate your data, if in error on our end however re-transmitting the data avoids this step.

Transmitting Data

You may transmit data to central database via email. Please note that the data reporting function is in transition and there will be limited support during this period including a pause in benchmarking reports.


This is the most reliable means to transmit:

  • On the main screen of the database, select "Export and Transmit Data" button.


  • Note the path in the field in the middle of the subsequent blue screen. By default this is probably  "C:\SepsisDB\Export\"


  • Be sure the box "Anonymize Data" has a check mark in it. If not, select the box to place a check mark in it.


  • Select the "Export Data" button. Your data will then dump into the folder identified in the path above(probably "C:\SepsisDB\Export\") in 3 files.


  • Go to the path where you data is stored. You will find 3 text files (chart.txt, abx.txt, chartdefaults.txt).



  • You will receive a confirmatory email from the "Out of Office" feature in Outlook that confirms your email was received.

Help Requests

Your IT department internally will likely be able to help with troubleshooting. Sometimes firewalls block installation. Note that in order to print reports Microsoft Excel Arial Fonts must be available on the printer.
